Camp, sports, daycare, school physical, & drivers license physical forms add up to thousands of forms we are filling out per month.

PA Emily and our office manager Christian can help you figure out what appointments your children need and get your entire family scheduled at a convenient time before the rush. They will be having dedicated phone hours 2-4 pm Mondays to Thursdays, so that they can carefully go over what your child needs, if they need a new appointment or if a recent physical counts for forms you need, etc.

If your child is going into Kindergarten, 6th grade, or 11th grade, the school will ask for physical forms & immunization records – even if they have not mentioned it yet. We will provide these for you at your child’s well visit in May and June so that you are ahead of the game.

If your child plays a sport, they will likely need the PIAA sports physical form filled out. Per PIAA, the physical counts if it is done after May 1st (although some schools may still follow the older guidelines of "after June 1st" – so just check your school’s athletic forms).

If your child will be starting college in the fall, find out what the college requires in terms of physical forms and immunizations. We will also help set you and your child up with our portal, in case they need a telemedicine visit while at college.
